About this service

We provide this service to people in their individual living environments after they have completed treatment on any of the Group’s in patient clinical care pathways. The service we provide will typically offer additional support with activities, including household tasks, personal care and any other activity that enables an individual to maintain both independence and quality of life. The length of time within this pathway could range from a few weeks through to lifetime support.

Within this pathway, the package of care is tailored for an individual and could range from regular visits through to live in care.  There is also an option to purchase therapy sessions as part of the discharge plan.
