Short-term assisted independent living pathway which promotes optimum patient independence prior to discharge

About Assisted Independent Living

We only admit a patient to the Assisted Independent Living pathway as an internal transfer from one of our acute care pathways. We offer this pathway following acute rehabilitation to focus on promoting a patient’s independence with the ultimate objective of a return to the home environment. We typically recommend a length of stay for up to 12 weeks.

Our accommodation directly supports the aim of living more independently, with rooms typically including a small kitchen area. We ensure that a patient can access up to four therapy services to help with the development of living skills including cooking and other household tasks. Our occupational therapists lead the treatment on this pathway, supported by the physiotherapy team.

We are also able to support the patient if they wish to engage with a future care provider.
